You can use this form to request a current cash-in (surrender) value of your policy.
Only use this form if:
- You want to know the current cash-in value of your policy
- Your policy is not a pension or an FMISA
- You’re the policyholder(s) or the policyholders’ regulated Financial Adviser and have already registered your authority with us.
Do not use this form if:
- You’re a pension customer
- You’re an FMISA customer
