I want to apply a Power of Attorney or Court of Protection Order to a policy

If you are unable to use the online form to apply your Power of Attorney or you need to apply a Court of Protection order, we’ve outlined below what you will need to provide to proceed.

We’ll need the following:

  • A copy of the full Power of Attorney document or Court of Protection Order (this doesn’t need to be certified)
  • Identification for the policyholder (or Donor) and attorney(s) (the individual(s) who is/are being given the power of attorney)
  • We’ll need you to upload Enhanced identification from List 1 and 2– please refer to the Online Identification Guide

If applicable, we'll also need a letter from the care home on headed paper, signed by the manager. This applies when the policyholder has moved into a care or residential home and will be accepted in place of Enhanced identification requirements. This letter must meet the following criteria:

  • Include the name and address of the care home
  • Be on the care home’s headed paper
  • Confirm the policyholder’s full name
  • Confirm the policyholder’s residency within the care home
  • Be signed by the care home’s manager or matron
  • Be dated within the last six months

Policies held in Trust:

Please note that if the policy is under Trust, the Lasting Power of Attorney provided does not allow the Attorney to carry out Trustee duties so its impact on this policy will be limited.

Most of the decisions within the Trust must be authorised by the Trustees acting together therefore the Trustees may want to seek advice as to whether it is appropriate to make changes to the Trust to allow future transactions to run smoothly.

You can use this form to apply your Power of Attorney to a policy.

Only use this form if:

  • You have your Lasting Power of Attorney (LPA) online access code, and it was issued within the last 30 days
  • Your Lasting Power of Attorney was registered after 17th July 2020

Please note: if the policyholder resides at a UK care home, we require a letter to be uploaded from the care home. This letter must meet the following criteria:

  • Include the name and address of the care home on their headed paper
  • Confirm the policyholder’s full name
  • Confirm the policyholder’s residency within the care home
  • Be signed by the care home’s manager or matron
  • Be dated within the last six months

Do not use this form if:

  • The Lasting Power of Attorney includes any Instructions
  • The policy is held in Trust
  • The Power of Attorney was registered before 17th July 2020
  • You’re submitting a Court of Protection Order
